Registration form for your pet for „The House and Pet Care“
The registration form is part of our care contract. In it, you note down the current state of health of your pet. The truthful information entered on the form serves as a basis for us to create the right “The House and Pet Care” package for you and your pet.
The medication schedule, which should be sent to us one week before the start of care, ensures that the medication status is up to date and that the medication has been properly prescribed by your vet. In this way, both the necessity and the dosage are checked again and, if necessary, updated by the treating vet.
Power of authorisation for veterinarian
If your dog or cat needs an unscheduled visit to the vet in your absence, please give us a power of attorney for visits to the vet. This releases the vet from his duty of confidentiality towards third parties for the period of care. This will allow you to speak openly about your pet’s medical history with the vet at “The House and Pet Care” if necessary.
General terms and conditions and data protection
Both the registration form as well as the General Terms and Conditions and the GDPR are mandatory for both contractual partners for the period of care.
